Continued Health Care Benefit Program

The Continued Health Care Benefit Program (CHCBP) is a premium-based plan that offers temporary transitional health coverage for 18 to 36 months after TRICARE eligibility ends.

CHCBP acts as a bridge between military health benefits and your new civilian health plan. It is a healthcare program intended to provide you with continuous healthcare coverage on a temporary basis following your loss of military benefits. Eligible beneficiaries

  • The service member (who can also enroll his or her family members)
  • Certain unremarried former spouses
  • Children who lose military coverage due to age

Purchasing CHCBP

You must purchase the CHCBP within 60 days of loss of TRICARE eligibility. CHCBP coverage begins on the first day after the loss of TRICARE eligibility.

Visit the TRICARE CHCBP page and complete the CHCBP Enrollment Application form (DD form 2837) to start the process.

Affordable Care Act - IRS reporting for 2016

For the tax year 2016, Humana Military will report your CHCBP healthcare coverage to the IRS. You will get IRS form 1095-B for proof of minimum essential coverage for you and/or your family members. You should receive the 1095-B form by the end of January 2017. The form should be kept with your 2015 tax records.
