TRICARE has limited coverage for the services included in the below policies.  Authorizations are required for Prime beneficiaries for these services, and benefit review is available for Select beneficiaries prior to the service being rendered to determine coverage. Please submit authorization requests through provider self-service, which can be accessed here.
